Machine RMA Repair Request
You must complete ALL STEPS to ensure that your machine is repaired quickly and correctly.
STEP 1: Fill out the form below completely and click “SUBMIT”. All fields, except line address 2, must be completed for the form to submit.
STEP 2: The next step is to complete the $0 invoice that will save your credit card on file. The credit card will be used AFTER the equipment is repaired for the return shipping fees and for any repair charges (if the repair is not covered by warranty, unit is no longer under warranty, or if the warranty has been voided).
STEP 3: Once submitted, you will receive an email within 1-2 business days that will include your RMA number and an address with instructions on where to ship your machine. DO NOT ship your MagnaWave machine until you receive the RMA instructions email. If you do not see this email in 48 hours please check your junk and spam folders.
STEP 4: Send your tracking number to us at Support@mwpemf.com so we can be on the lookout for your machine.
