Machine Repair Request – Maxx, Maia, Semi 3 & 5, Semi 5, Semi 5 Duo (Vesta Duo), B2

If your machine needs to be repaired please fill out this form you will receive an email with an address to ship machine and a repair # as well as shipping instructions.

You must complete ALL STEPS to insure that your machine is repaired quickly and correctly.

STEP 1: Fill out the form below completely and click “SUBMIT”.

STEP 2: Once the below form is completed you will need to complete the credit card authorization form on the next page.

STEP 3: Once both forms have been submitted, you will receive an email within 1-2 business days that will include your RMA number, and an address with instructions on where to ship your machine. DO NOT ship your machine until you receive the RMA instructions email. If you do not see this email in 48 hours please check your junk and spam folders.

  • Your machine Serial Number is located on the lower corner on the inside of the door of all All-Terrain and Traveler machines. It is on the bottom of the office machines. It is usually a 5 digit number. This number is required before your machine can be repaired.
  • Be as descriptive as possible, so we can diagnose and repair your product quickly.
  • When your machine is repaired where would you like it shipped? Someone must sign for the machine when it arrives back to you.
  • You MUST fill out the credit card authorization form on the next page in order to receive your RMA email. If machines are available, you may rent one during your repair time. The cost is $100 a week + shipping. If you would like to check the availability of a loaner machine please or your machines warranty status call 833-MagnaWave. By submitting this form you agree to all our terms and conditions including but not limited to the information below. MagnaWave and its suppliers are not responsible for lost or stolen packages or damages caused by return shipping. Loaner machines must be returned promptly upon return of your machine, otherwise you may be charged additional fees. MagnaWave is not responsible for returning any machines by a certain date, if you are in a rush please choose expedited return shipping. Depending on repairs needed, machine service can take 2 to 7 business days + shipping. You are responsible for all shipping charges, warranty repairs will be covered.
  • VERY IMPORTANT - TO COMPLETE REPAIR REQUEST CLICK SUBMIT - Then you will need to complete the form that is emailed to you. If this form is not completed you will not receive your RMA and your repair time may take longer.
