You MUST fill out the credit card authorization form on the next page to receive a RMA for service or repair. If you would like to check the warranty status of your machine or the availability of a loaner machine, please call 833-MagnaWave. If a machine is available, you may rent one during your repair time; the cost is $100 a week plus shipping. By submitting this form, you agree to all of our terms and conditions including but not limited to the information below. MagnaWave and its suppliers are not responsible for lost or stolen packages or damages caused during return shipping. Loaner machines must be returned within 3 business days upon return of your machine, otherwise you may be charged additional fees. MagnaWave is not responsible for returning a machine by a certain date, if you are in a rush, please choose expedited return shipping. Depending on repairs needed, machine service can take 7 to 10 business days plus shipping. You are responsible for all shipping charges; repair costs for equipment covered under warranty will be covered.
My machine will be cleaned before sending in for repair. If not, a minimum fee of $125 will be charged for cleaning services.*
